Acceptance of Terms
By downloading, accessing, or using the Well-Check mobile application (“App”) or the Well-Check website (“Site”), you agree to be bound by these Terms & Conditions (“Terms”).
Overview of the Service
Well-Check provides a scheduled wellness check-in tool that allows users to send notifications to designated emergency contacts if a check-in is missed or if the user indicates they need assistance.
Important:
Well-Check is not an emergency response service, medical provider, or 911 substitute.
If you are experiencing an emergency or are in immediate danger, call 911 or local emergency services.
Eligibility
To use Well-Check, you must:
- Be at least 18 years old, or have the consent of a parent/guardian.
- Create an account with accurate personal information.
- Agree that you are responsible for the accuracy of the emergency contact information you provide.
User Responsibilities
You agree to:
- Use the App only for lawful purposes.
- Provide accurate and up-to-date contact information.
- Ensure your device allows notifications so check-ins can function properly.
- Understand that missed notifications may occur due to connectivity or device issues.
You are solely responsible for choosing contacts who are willing and able to respond if alerted.
Emergency Disclaimer
Well-Check does not monitor check-ins in real time and does not contact emergency services directly.
Well-Check simply automates alerts to the contacts selected by the user.
The App does not guarantee:
- That contacts will receive alerts,
- That contacts will respond,
- Or that help will arrive.
The user and their designated contacts are entirely responsible for response actions.
Subscriptions & Payments
Some features may require a paid subscription.
By subscribing, you authorize Well-Check, LLC or our payment processors to charge your payment method on a recurring basis until canceled.
All fees are non-refundable except where required by law.
Account Security
Users are responsible for maintaining the confidentiality of login credentials.
Well-Check, LLC is not liable for unauthorized account access resulting from a lost or compromised device or password.
Intellectual Property
All trademarks, logos, graphics, text, and software associated with Well-Check are owned by Well-Check, LLC. You may not copy, modify, distribute, or reproduce any part of the App or Site without prior written consent.
Privacy
Your use of Well-Check is also governed by our Privacy Policy, which explains:
- How data is collected,
- How it is stored,
- And how it is shared.

Limitation of Liability
To the fullest extent permitted by law, Well-Check, LLC is not liable for:
- Any direct, indirect, incidental, or consequential damages,
- Any failure of alerts, notifications, or contact responses,
- Any emergency situations or outcomes related to the use or non-use of the App.
Your use of the App is at your own risk.
No Medical Advice
The App does not provide medical advice, diagnosis, or treatment.
Consult a qualified healthcare professional for medical concerns.
Termination
We may suspend or terminate access to the App at any time for violation of these Terms or for fraudulent or abusive activity.
Governing Law
These Terms are governed by the laws of the United States. Any disputes will be resolved in accordance with U.S. law.
